Join our dynamic Hospital Social Work Team at the London Borough of Hounslow. Work in a multi-disciplinary environment to ensure safe discharge and follow-up care for Hounslow Residents leaving the hospital.
Participate in MDT discussions, Mental Capacity assessments, Best Interest meetings, family meetings, and safeguarding concerns. Contribute to the team's duty rota and uphold human rights in daily roles.
Experience in a Hospital or social care setting. Knowledge of legislation such as the Care Act 2014 and Mental Capacity Act 2005. Ability to work in a fast-paced environment and manage priorities effectively.
