Job Description

Join our Customer Enhancement Service team as a Customer Enhancement Service Advisor. Conduct telephone assessments, attend home visits, construct care plans, manage case load, implement knowledge of specialist services, offer support, and deliver training.

Responsibilities

  • Conduct telephone assessments
  • Attend home visits (when appropriate)
  • Construct individual care plans
  • Advise on reasonable adjustments
  • Work with team members to ensure customer support
  • Manage a case load

Qualifications

  • Certificate/Diploma in Counselling/ Psychotherapy
  • Care Certificate or equivalent experience
  • Foundation Degree in Health and Social Care/Mental Health
  • Occupational Health Nursing
  • Experience in care/health/information and advice
