Join our busy Community Mental Health Team for Later Life as a CPN/Care Coordinator working with older people living with dementia or functional illnesses.

Responsibilities

Care Co-ordination under CPA
Complete Memory Assessments (training provided)
Administer depots
Complete initial assessments
Participate in Duty Rota
Carer Assessments

Qualifications

Community Psychiatric Nurse qualification
Experience working with older people
Commitment to professional development
Motivated and compassionate
