Job Description

An excellent opportunity has arisen to recruit a Community Psychiatric Nurse/Care Coordinator into our busy Community Mental Health Team for Later Life. We are looking for an experienced, committed, and motivated professional to work with older people living with dementia or functional illnesses.

Responsibilities

Care Co-ordination under CPA
Complete Memory Assessments (training will be given)
Administering depots
Complete initial assessments
Participate in Duty Rota
Carer Assessments
