Responsibilities

  • Undertaking Care Act 2014 Assessments - identify individuals’ care and support needs, ensuring a personalised approach that emphasises the best outcomes
  • Completing Care Act 2014 Reviews - evaluating the strength and suitability of an individual's care and support plan, ensuring that it continues to meet their needs and preferences
  • Managing Safeguarding enquiries - investigating concerns about potential abuse or neglect, ensuring the safety and well-being of vulnerable individuals
  • Mental Capacity assessments - determine an individual's ability to make specific decisions regarding their care and treatment
  • Undertaking duty on rota basis
  • Undertaking DSTS and CHC
  • Court related work

Qualifications

  • Registered with Social Work England and hold a degree in Social Care or similar
  • 2 years post ASYE working experience as a UK registered social worker
  • Experience of working in a multidisciplinary team setting
  • Experience in undertaking complex Mental health and Physical health cases
  • Sound knowledge and ability to deliver social work support to service users and their families
  • Ability to communicate complex social issues clearly to both patients and professionals
  • Ability to plan and coordinate social care plans and aspects of patient discharge
